Post-Secondary Funding Application Form

K’omoks administers funds that it receives from Indigenous and Northern Affairs Canada’s (INAC-Formerly AANDC) Post-Secondary Student Support Program (PSSSP) and the University & College Entrance Preparation Program (UCEPP). Both programs provide funding support for tuition, books & supplies, and a living allowance. Living allowances can only be given to those attending full-time studies.

POST-SECONDARY STUDENT FUNDING CONTRACT

This contract is between the K’ómoks First Nation and the undersigned student for the purpose of receiving financial assistance from the K’ómoks First Nation in order to pursue post-secondary studies.

I have read the K’ómoks First Nation Post-Secondary Policy. I understand its contents and intent and therefore I agree that:

I will complete the K’ómoks First Nation Post-Secondary Funding Application Package and return it to the K’ómoks First Nation Education Coordinator.
I will provide a photo-copy of my current status card.
I understand that my application will be reviewed according to criteria within the K’ómoks First Nation Post-Secondary Policy and available funding.
Once my application has been approved, the Education Coordinator will send a sponsorship letter for tuition and compulsory student fees to the post-secondary institution. Tuition and student fees will be paid directly to the educational institution.
A sponsorship letter will also be sent to the bookstore for required books and supplies. If the Bookstore does not accept sponsorship letters, an advance will be provided to me with the understanding that all receipts must be provided to the K’ómoks First Nation.
If I am a First year student or on Probation by the Institute, to be eligible for the monthly living allowance, I will submit the Confirmation of Attendance and Progress Forms to the Education Coordinator by the 25th of each month.
I will be responsible for providing a copy of my transcript for all sponsored courses within 45 days of the semester ending. Failure to provide a transcript will result in an immediate end of all financial assistance from the K’ómoks First Nation.
If there are any changes in my educational plan, I will inform the Education Coordinator in writing as soon as possible.
If I am not able to complete the courses or program that I have entered into without an approved reason, I will repay the K’ómoks First Nation for all funds that have been paid towards my education from the dates mentioned in the corresponding Post-Secondary Funding Application Form.
I understand that until all outstanding debts to the K’ómoks First Nation have been paid, I will not be eligible for post-secondary funding.
